Covid RAt tests

Riverwood Community Centre is a non-for-profit Community Centre serving the community for more than 40 years. 

The government has funded us to provide RAT tests to the community and as part of this support we are asked to register some details from the people that are given this tests. 

All the information we collect is confidential and will not be shared with any other organisations or services, except for the Department of Community Services, which is also optional. At the ending of this form you will have the option to either share this information or not with DCS. 

The free rapid antigen tests (RATs) are available every Thursday from 9am until 12 noon to:

People with disability, immunocompromised people, and their carers

Eligible Commonwealth Concession Card holders in NSW:

Pensioner Concession card

Commonwealth Seniors Health Care card

Health Care Card (including Low Income Health Care card)

Department of Veterans’ Affairs Gold, White or Orange cards.

For people with disability or immunocompromised, this may vary depending on their need, it’s a total of eight RATs per person per month. Concession Card holders can access 10 Rapid Antigen Tests over three months.

Covid Test Form is required to be completed.
