Riverwood Community Centre’s Aged Service delivered high quality, targeted Commonwealth Home Support Program (CHSP). The Commonwealth Home Support Programs are for people who are frail aged and their carers and are aimed at helping people to remain living independently in their own homes for as long as possible.
Our highly qualified and trained team provide and deliver a broad range of services to the aged community. They follow a ‘Wellness Approach’ that focuses on the assessment, planning and delivery of services that build on a participants’ strengths, abilities and goals. This is a responsive model that gives the participants greater choice and control in the services that they receive, enabling participants to be at the center of the decision making process.

In order to access our services, participants must be above 65 years old and registered with My Aged Care. Upon registering with My Aged Care, request to be referred to Riverwood Community Centre.

Fees are a contribution towards the cost of providing the service. All fees charged by the Aged Service are approved by the Riverwood Community Centre Board of Directors and are reviewed regularly. Clients will be given information regarding fees during assessment. If a client has difficulty in paying a fee, this can be discussed with the Service Coordinator.
Even though people may be eligible to receive a service, sometimes we may not have the resources to be able to help straight away. If this happens we will place clients on a waiting list. We review waiting lists on a regular basis and will let clients know how long they are likely to wait before service can begin. Priority is given to people with the greatest need, to those who live alone and to those who do not have family members to assist them.
Information and Referral
The Service has a large resource library of information and staff can provide information about, and referral to, a wide range of other community services and resources that are available in the area.
Please note: clients who receive a Community Aged Care Package may not be eligible for some of the services described.
